Q Do I have to be an Indian citizen to become an EVALIFE Health Card member?
A Yes. EVALIFE Health Card is available to anyone residing in India.
Q What is the validity period of this card?
A It is valid for one year. The membership card has to be renewed annually.
 
Q How long does it take for activation of my membership card?
A Your membership card becomes effective after 24hrs of your receipt of the card. Showing the card to healthcare provider is mandatory.
 
Q If I change my address or other information can my spouse call and change our information?
A Only the primary card holder can make changes on the membership.
 
Q Does EVALIFE Health Card sell my personal medical information to other organizations?
A No. The database of members is used only to share information related to health care services, health updates, articles, news and promotional offers among the member group only.
 
Q How much do I have to pay when I go to the Doctor?
A The saving varies from one provider to another. The savings are determined by the agreement we have with the service provider.
 
Q The listed provider is not giving me discount as listed on EVALIFE Health Card website. What can I do?
A This is unlikely. But, in case you face such a situation, kindly call us so that we can speak to the concerned authority at the provider's outlet. Certain provider may refuse discounts and value additions on already discounted rates for cashless insurance services as per the agreement with the TPA/Insurance company.
 
Q If I am not happy with the service provided by your service provider, is EVALIFE Health Card liable?
A We at EVALIFE Health Card, have made a conscious effort to network only with quality healthcare service providers. Since we are only a facilitator, we cannot and do not monitor the quality of services of the provider and such responsibilities are of the service provider. All disputes related to services of providers should be directly dealt by the patient only.
 
Q Do I get insured if I become an EVALIFE Health Card member?
A EVALIFE Health Card is not an insurance. EVALIFE Health Card does not reimburse any healthcare expenditure. EVALIFE Health Card does not offer any cashless or third party administration services.
